Scaffolding Firm Fined Following Fatal Five-Metre Fall of Trainee
By Safety Desk · 07 November 2026
Ask Scaffolding Ltd has been fined after a trainee scaffolder died from a fall at a North Yorkshire housing development where safety guidance was ignored.
A scaffolding contractor based in North Yorkshire has been sentenced following the death of a trainee worker who fell five metres from a partially constructed scaffold. The incident occurred in July 2023 at a residential building site near Ripon when the operative stepped on an unsecured board and fell to the ground.
An investigation by the Health and Safety Executive (HSE) revealed that the company, Ask Scaffolding Ltd, failed to establish 'Scaffolder’s Safe Zones' or ensure the use of essential fall protection equipment. Despite industry requirements for collective protection measures, witnesses reported that operatives frequently worked without using harnesses or lanyards, and guardrails were missing from several locations on the site.
The HSE emphasised that when collective protection measures are not sufficient to create a safe zone, personal fall arrest equipment must be utilised. The investigation concluded that the firm had systematically failed to follow safety guidance, leading to the fatal incident.
Ask Scaffolding Ltd pleaded guilty to breaching the Work at Height Regulations 2005. At Leeds Magistrates’ Court, the company was ordered to pay a £16,000 fine along with £8,000 in costs. This case serves as a critical reminder for site managers and procurement teams to verify that scaffolding partners strictly adhere to SG4 guidance and provide adequate fall protection PPE for all staff.
